Email box not showing up

I cannot get the email box to show on the guestbook. I have indicated Require E-mail Yes, but it does not show on the form.
I am using the latest version of Phoca Guestbook 1.3.2 with Joomla 1.5.15

Never mind.... I was changing it ONLY in parameters within Phoca.. did not make changes in Menu...
