Question 1:
the "title" in front of the inputbox Title is not shown as "title", but as:

How can I solve this?

I change already the table width , but it stills go wrong

I don't get it in standard browsers, but for non-standard browsers try to define the width of the <td> element in the table:

components\com_phocaguestbook\views\phocaguestbook\tmpl\default.php

it should help... if not there can be problem with space on your site (there must be some specific width for tinyMCE editor because of menu items so there is no more space for title (maybe)....
