[solved] Phoca Maps: "Get Route" stop works

Good morning,
can you help me, please?
My "Get Route" on Phoca Maps has stopped his works.
I Have tride to uninstall and reinstall component and plugin, but nothing has changed.

Solved!
the problem occurs when the same page will also display a form created with "form maker pro".
I leave published this answer, perhaps can serve to someone else
