Joomla 3.2 Phoca 4.0.4 - thumbnail creation errors

I am just trying to use this product on one of my websites and have found that many of the thumbnails are not created.

On the categories view page although I get _l_, _m_ and _n_ thumbnails the view of the system is looking for _m1_ _m2_ and _m3_

On the actual category view page I seem to have random results with any refreshing showing missing image thumbnails some of the photos (and the list of photos without thumbnails being shown is different on every refresh).
