I have the same error. I found out that in the upload form wrong ampersand in action url ("&" - "&").
Example: action="*****/index.php/2013-04-14-10-54-00/category/1-cat1?tab=1&task=upload&3866a1901e902acd2343504a6c131b52=7nh06bbfqfit9tml41fl6s63e4&55ce218f69176f0f5f2b097ee2a68e8a=1&viewback=category&tab=1"
How can I fix it?
