Category with wrong hyperlink

After upgrading Phoca Gallery from 2.8.1 tot 3.2.1
(export database from 2.8.1, import database to 3.2.1)
We have next problem:

Some categories in the root generate a 404 Not Found error
The hyperlink for that category is missing the "index.php"

For example, category with correct hyperlink:
http://www.mydomain.nl/index.php/fotos/ ... y/337-2012

Category with wrong hyperlink:
http://www.mydomain.nl/fotos/category/293-2011

This results in error:
The requested URL /fotos/category/293-2011 was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

How can we correct this?

Hi, there were no changes regarding URL creating, URL are still created by Joomla! itself, try to recheck your SEF settings.
