The Upload Dialog

How can I remove multiple fields from the user upload dialog? Specifically, I don't want/need Author, Author's Email, Author's Website, License, and Version. I want them gone.

When a user goes to my Uploads page (it's a very simple implementation) the page shows the word "Uploads" in smallish type, followed by a gap of about 1", followed by a tab with the word "Uploads" in it, followed by the framed upload dialog box. How can I remove everything EXCEPT the framed upload dialog box?

I like to erase the blue marked content on the left (like the thread opener described). Can you tell me the php file/line to edit ?
