1,2) - seems like some table or some column is missing in your database

3) did you select the file for playing in editing of file?

4) Phoca Download only gives information from server and file to browser and then the file is downloaded by your os, browser - server communicatio. Try to check if there are no limitiations on your server (e.g. limit for downloading, etc.) Try to use other os/browser.
