Images are not showing up.

I have two questions:

Why cant I add images from my media gallery per Joomla?

When I upload images from "Select Filename" I don't see anything but a white box?

FTP
You need to upload images into the images/phocagallery directory in your server. Phoca Gallery will automatically create thumbnails of these images when you add them to a category.

Q2.)
Enable Thumbnails Generation needs to be set to Yes
backend --> Components --> Phoca Gallery --> Control Panel --> Options --> Thumbnails --> Thumbnail Generation Settings --> Enable Thumbnails Generation=Yes
