Unable to upload an image from the back end

Hi all!

I am not able to add any image because the "select filename" button doesn't work, the image Filename window doesn't load properly, just staying blank. I tried different browsers and machines to no avail. I eventually updated Joomla to 3.2 and Phoca to 4.0.4. but this didn't help either. Please share your ideas...
