PhocaDownload Extenssion / Hack : survey email before downl

Hi -

I installed the hack and I'm pretty sure I followed the instructions, however when I choose a chronoform form for the license text, it no longer includes a download/submit button.

So now basically when you click on a phocadownload file to download, you are redirected to a page with form fields, but cannot sent submit them anywhere. Also, the "License Agreement" heading is not coming up on the page.

Is it possible that this has something to do with how I created my chronoform? Not much documentation in the hack files or on chronoforms website.

I know the last post on this topic was quite a while ago, but if anyone has successfully installed this and has gotten it to work, I would be grateful for any help! If it's easier, you can email me at laurin.paige (at) gmail (dot) com
