No, I uploaded an image to my site via Phoca Gallery and it will only show the image if it is set published and authorized for the "main categories" like incidents, training and certification, etc. or else it will not show it.

mmmmm, but I can see images representing the category 'incidents' and all subcategories of 'incidents'.
Try to clear your browser cache.

The only way I can get it to work is if I have the image authorized for the categories view.

Sorry, no more ideas. I can not reproduce the error in my test environment and therefore do not know how it can be eliminated.The only way I can get it to work is if I have the image authorized for the categories view.
