Adding Fields and Reformating Display

Hello ---
Is there any documentation that discusses how to add new fields to the guestbook form / database. It is common to provide a Location field in many guestbooks, so that the signer can tell people where they are from. I also would like to add an email link to the person's name, as opposed to displaying the actual email.
