How to copy queries and run in phpmyadmin

Hi,
I have the guestbook installed on the trial site on my local machine and there everything is fine, (and it's a very nice component which I'm grateful is available for us to use). There, the installation worked well in the normal way, so I am at a loss to see why I'm now having problems.
The problem is that I installed it OK on the live site - worked fine. Then I changed a parameter (registered only or public) and the change did "not take", i.e. the guestbook still wanted registered members only although there is nowhere to register.
I then removed the component and installed it again, but this time no tables were created. I have read the instructions, but I'm sorry, I have never worked with mysql or phpmyadmin before and I would be very grateful if you could walk me through the procedure, i.e. where to enter the copied lines from install.bak, etc.
Also the instruction says not to copy ALTER TABLE queries, but I can't see that text anywhere in install.bak. Only CREATE TABLE and DROP TABLE IF EXISTS.
