If you have the possibility, try to enable error reporting in your php to get the information about possible error

I have the very same problem and I guess it's not solved yet?

For testing I installed 2 different templates and PHP error reporting on (E_ALL & ~E_NOTICE).
on template#1 (YooTheme) no problems!
on template#2 (GavickPro) a blank page and no PHP errors.

Is the Guestbook in a way dependent on certain scripts that cause problems with scripts used by the template?

... minutes later I found the cause:

In the menu linkdescription it says:
index.php?option=com_phocaguestbook&view=phocaguestbook

But selecting ther menu (submenu in this case) the URL will be:
<domain>/index.php?option=com_phocaguestbook&view=phocaguestbook&id=1&Itemid=34

This results in a blank page, but removing the "&Itemid=34" parameter fixes it!
How can this be fixed quickly? I mean, can I surpress the addition of the ItemID parameter or can de code of Guestbook be easily fixed?

index.php?option=com_phocaguestbook&view=phocaguestbook&id=1&Itemid=34 ... this is the right link, the itemid need to be there because of menu link settings (parameters) ... but I have no idea why there is a problem with the Itemid, ... is the template made directly for Joomla! 1.5 native ?

Yes, it's a J1.5 native template which I bought at gavick.com called MusicTop.
I can send it to you for test purposes.
