Default in description field when adding images

When I use the image upload function in the list of images (Category view) I have two issues:
1) Is it possible to put the following in the description field as a default to ease the capture of additional information relating to the image:

Artist:
Title:
Medium:
Size (in cm):
Price:

2) The second issue is that the image does not appear once it has been uploaded (unless the Display Num is altered), how do I make the image show immediately?

2) can you paste a screenshot of what do you exactly mean with "display num is altered)?

1) there is no such option, so the only one way for this is customizing it in the code directly

I have fixed the description field problem.
I do not seem to be able to post an image on imgur, either I get 'you cannot use this phone number' or 'you are registering too quickly'.
So please enable image so I can show you the screenprint.

The problem is that the user gets no confirmation that the image has been successfully loaded. So I have turned on the parameters as follows:

...and a Display Num select box appears which can be updated thereby showing the latest upload. This does not seem right.

Thank you Christine for the HTML5 uploader idea but my screen still does not refresh to show that the images have loaded, and I now cannot add a different description for each image.
