I need to add in the alt text attribute of the download buttons the name of the file or the title of the file for screen readers. Anyone know how to accomplish this?

Hi, not sure what you exactly mean "for screen readers"?
In any case, I think, this needs to be customized directly in php file (plus maybe the CSS)
e.g. for category view:
components/com_phocadownload/views/category/tmpl/ ...
